Job Summary

We are seeking a compassionate and experienced individual to fill the role of Community Health Worker Practitioner (CHWP) at the Community Outreach Team (COT) program, affiliated with the Center for Nonviolence and Social Justice at Drexel University's Dornsife School of Public Health. The successful candidate will split their time between providing support at partnering community organizations, and remote work, ensuring the delivery of comprehensive community health services. This job requires a significant amount of community-based work (travel), and a willingness to engage with a diverse range of neighborhoods in Philadelphia.

Essential Functions

  • Outreach and Engagement: Conduct in-person outreach and engagement activities at various partner sites, including New Foundations Charter School, Students Run Philly Style, and other youth-focused organizations. Develop trusting relationships with potential clients based on lived experience. Assist in identifying and participating in at least 1 resource fair and other tabling event per month.
  • Case Management: Provide comprehensive case management services, including scheduling and accompanying participants to appointments, navigating systems, and setting personal goals. Advocate for unmet case management needs of participants.
  • Group Facilitation: Coordinate and implement trauma recovery groups, such as the CYPHER groups, which integrate trauma education and coping skills. Provide support and assistance in group settings, emphasizing empowerment and skill-building.
  • Documentation and Reporting: Maintain accurate records of patient interactions, program activities, and outcomes. Ensure compliance with funding requirements and organizational guidelines.
  • Team Collaboration: Collaborate with HHP and hospital staff to develop appropriate interventions, including warm handoffs and referrals. Provide ongoing education to non-peer team members to better understand participants' unique experiences and needs.
  • Community Education/Engagement: Participate in community events and presentations to educate Philadelphia citizens and professionals about HHP and trauma.

Required Qualifications

  • HS Diploma or GED
  • Certified Peer Specialist (CPS) Certification in the State of Pennsylvania required.
  • Minimum of 1 years’ experience providing peer services to violently injured people.

Preferred Qualifications

  • Experience in facilitating groups preferred.
  • Proficiency with Microsoft Office and Excel preferred.
  • Valid Driver's license preferred.

Additional Information

This position is classified as Non-Exempt, grade G. Compensation for this grade ranges from a Minimum: $ 15.58 - Maximum: $ 23.37 per hour. Please note that the offered rate for this position typically aligns with the minimum to midrange of this grade, but it can vary based on the successful candidate’s qualifications and experience, department budget, and an internal equity review.
