Description:
The Community Health Outreach Worker will educate members of the community about the importance of early detection and screening and will navigate community members to screening services. The Community Health Outreach Worker will also participate in community events and develop and foster partnerships with University of Illinois Cancer Center community partners, per the aims of the Community Engagement and Health Equity (CEHE) Office.
Duties & Responsibilities:

  • Assist and support the Associate Director and Director in community-wide or in-house engagement service programs by providing information regarding the UI Cancer Center and UI Health health services, program requirements, procedures, and navigates community members to the local agencies or organizations for assistance based on identified needs.
  • Assist in conducting surveys such as community health needs assessment sand informs the office of the needs of the communities in the Cancer Center catchment area of Cook County, as required by the CEHE office.
  • Assist in program-wide recruiting efforts by developing relationships with community-based leaders, organizations and community members and providing approved health education materials and resources to community members and organizations.
  • Collect information from community members and program participants for reports and data to measure program impact. Assist with preparing reports.
  • Maintain complete and accurate records in accordance with CEHE procedures. Document all community events and enter data into REDcap.
  • Represent the UI Cancer Center and CEHE Office at community events and coalition/community meetings to understand community issues and build relationships with community members.
  • Assist in establishing new CEHE/Cancer Center programs and services as needed.
  • Serves as a liaison with other units within the UI Cancer Center, on campus and external partners.
  • Attend and successfully complete all required training programs. Participate in ongoing conference calls, webinars, and other professional development opportunities.
  • Perform other related duties and participate in special projects and assignments as assigned.

  • Minimum Qualifications Required:

  • High school diploma or equivalent.
  • One (1) year (12 months) of experience (paid or unpaid) in work related to the job to be performed.
