Physical Requirements
The physical requirements for this position include regularly sitting at a computer workstation for extended periods of time with repetitive motion (such as typing); walking including pushing, pulling, and/or lifting up to 10lbs and occasional pushing, pulling, and/or lifting objects up to 50lbs. Occasional driving of company vehicle to transport patients as needed.

Monday – Friday, Day Shift; 8:00 am – 5:00 pm (40 hrs/wk)
Job Summary
This position is located off-campus in Hazard, KY.
The primary responsibilities for the Community Health Coordinator will involve one on one interaction with clients by outreach contact and reporting, independent case management efforts to reach families regarding children’s dental status, and navigation with transportation services to ensure timely access to dental care.
The CHW advocates on behalf of the client/family to dental providers and agencies. The CHW will complete home visits. They will be expected to promote individual capacity for improved dental health, provide focused health education information and materials to patients.
This includes preparing detailed spreadsheet reports, making multiple attempts to contact families, and facilitating necessary steps for treatment completion by the end of each summer. Additionally, the role encompasses diverse assignments such as community outreach, education, and support activities, contributing significantly to enhancing dental care accessibility for children in the community.
This position will require a valid driver’s license and includes transportation duties of clients as needed in support of care access.
Skills / Knowledge / Abilities
Valid driver’s license. Ability to use basic desktop applications such as Excel; Outlook; TEAMS, EPIC (electronic health records)

Preferred Education/Experience
Previous experience in administrative and/or clerical roles; prefer work experience related to community health outreach and/or program support/coordination. Prefer 2 or more years of experience working in a dental or dental outreach setting, or 2 or more years of experience working in case management or patient navigation services.
Preference will be given to a candidate who holds community health worker certification (CHW). KY Community Health Worker Certification (CCHW) at time of hire or would like it obtained within 1 year from date of hire.
