The HARBORVIEW MEDICAL CENTER PHARMACY DEPARTMENT is seeking a highly-motivated FULL-TIME ANTIMICROBIAL STEWARDSHIP PHARMACIST (DAY SHIFT). The Antimicrobial Stewardship (AMS) Pharmacist assists with the day-to-day activities of the HMC Infectious Disease (ID) and AMS Programs alongside a team of ID attending providers and a Lead AMS Pharmacist. This position will collaborate with a multi-disciplinary team to evaluate trends and implement institutional guidelines for the appropriate use of antimicrobials and will participate in designing safe, cost-effective therapies and provide education to health-care practitioners. 

PRIMARY RESPONSIBILITIES:
• Assist the AMS Physician Coordinators and Lead AMS Pharmacists with implementation of institutional guidelines for appropriate use of antimicrobials.
• Round with selected infectious diseases consult teams regularly to ensure appropriate antimicrobial use and successful transitions for outpatient antimicrobial therapy.
• Serve as a resource and support the HMC outpatient parenteral antimicrobial therapy (OPAT) program.
• Attend microbiology plate rounds.
• Improve antimicrobial prescribing practices through education of healthcare providers and pharmacy staff.
• Evaluate trends of antimicrobial resistance and usage throughout the medical center.
• Establish a system to monitor and provide feedback on the usage of antimicrobials in HMC ambulatory clinics.
• Participate in medication use evaluation and organizational process improvement projects.
• Primary preceptorship of pharmacy residents and PharmD candidates.
• Participate in the PGY2 ID residency program.

Strong candidates will have a completed PGY 2 residency. Infectious Diseases training is preferred OR a minimum of two years of infectious disease or stewardship experience in a clinically oriented hospital pharmacy practice.
REQUIREMENTS:
Two years of experience in pharmacy practice.  Months spent in the completion of an American Society of Health System Pharmacists accredited residency training program may substitute month-for-month for the experience.

LEGAL REQUIREMENT: State of Washington Pharmacist License.
