Job Description:

Community Health Education

Uses state approved materials to provide immunization education to diverse audiences. Maintains files on volunteers, families contacted, program materials, and related information. Collects demographic data and health and wellness data from families and participating community-based organizations. Evaluates knowledge gained and behavioral changes through approved evaluation tools. Facilitates group activities and workshops, and prepares charts, visual aids, and other teaching materials. May assist County Extension Agents by assuming responsibilities as assigned by the program director. Lifting and carrying teaching materials and/or supplies up to 40 lbs. May require standing, stooping, pushing and/or pulling. May require standing for prolonged periods both indoors and/or working outside to support programming.

Community Engagement

Actively recruits community partners and fosters existing relationships with a goal of reaching participants and expanding programming. Locates and recruits volunteer leaders for health extension program. Makes visits into neighborhoods to introduce programs to diverse audiences. Works closely with community partners to implement systems and environmental level changes. Markets the program through health fairs and other locations where a booth or display is appropriate.

Travel

Stays within assigned travel allocation while traveling to teaching sites from the official headquarters in Gainesville. Travel is primarily by means of a state vehicle. If a personal vehicle is used for travel, mileage will be reimbursed monthly at the current state rate.

Training

Develops and improves knowledge of vaccine-preventable illnesses to become more proficient in determining client needs; understands various methods of reaching and collaborating with the clientele, and develops proficiency in behavioral and systems change via participation in orientation; attends required trainings, engages in continuous on-the job training, and self-study. Completes community health worker training within 3 months of hire date.

Reporting

Maintains accurate records and reports on clients and personnel requirements.  Submits reports on time for supervisor or other designee.  Works with agency staff in utilizing records, reports, and other resources to assist in program implementation and in evaluating program impact. Should be comfortable using Microsoft Office Software.

Supervision

This position is supervised by the Program Director. Verbal and written instructions for training purposes and procedural forms.  Additional instructions on an as-needed basis.  Support and direction from state and local office. This position does not supervise others.

Communication 

Frequent contact with state office team, community organizations, and program participants.

Statement of Responsibility for Confidential Data

This position will have access to participant self-reported data, including contact, demographic, income, social, and health status information. 

Normal Work Schedule 

This position is full-time, up to 40 hours per week. Hours scheduled Monday through Friday, between 8 am – 8 pm. A moderate number of evening, early morning, and weekend hours are required due to the nature of the program.

 

 

Expected Salary:

$15.00-$18.00 per hour

Commensurate with education and experience. This position does not offer medical, dental, vision, vacation, sick, holiday, or retirement benefits. 

Minimum Requirements:

A high school diploma

Preferred Qualifications:

Able to support community-based health and wellness program.  Knowledge of healthy lifestyle behaviors and community engagement experience desirable.  Able to follow directions and complete reports.  Experience working with diverse clients helpful. Ability to work both as a team member and independently preferred.

Special Instructions to Applicants:

Community Health Worker Training required.

Valid driver’s license and reliable daily transportation for business travel required.

Section 435 Background check required.
